Spironolactone is a competitive inhibitor of aldosterone, and the drug itself has no activity. The diuretic effect occurs due to competitive antagonism with endogenous aldosterone, and it is a potassium-preserving diuretic.

Pharmacological action of spironolactone.

Since this drug only acts on the distal tubules and collecting tubules, and has no effect on other segments of the renal tubules, it has weak diuretic effect, slow onset and long maintenance time. It is mainly used for refractory edema accompanied by increased aldosterone, such as cirrhosis ascites, congestive heart failure, nephrotic syndrome, etc. It has a good effect on congestive heart failure patients with secondary increased aldosterone caused by sodium deficiency. The diuretic effect of this product is often poor when used alone, and often combined with hydrochlorothiazide or dihydrochlorothiazide can enhance the efficacy. In addition, it is used in the treatment of primary aldosteronism and essential hypertension.
