Nirendipine is a second-generation dihydropyridine calcium antagonist, which is the most clinically studied among the four common dihydropyridine calcium antagonists (nifedipine, amlodipine, felodipine), and is mainly used for the treatment of hypertension and coronary heart disease in the elderly. We know that the current routine treatment for hypertension is to combine guanethidine sulfate (antihypertensive drug) with dihydroquemide (diuretic drug). On the one hand, dihydroquemide reduces blood volume and cardiac output through sodium diuresis, and on the other hand, it can directly inhibit vascular smooth muscle, dilate blood vessels, reduce peripheral vascular resistance and lead to a decrease in blood pressure. Therefore, combined with guanethidine sulfate can enhance the antihypertensive effect and reduce the occurrence of edema. However, dihydrocresuloxide discharged more potassium, easy to cause hypokalemia, and if the combination of nirendipine and dihydrocresuloxide can avoid hypokalemia. Because this product is excreted by sodium in the urine, it not only helps patients to reduce blood pressure, but also has the effect of inhibiting the release of aldosterone, so it does not cause hypokalemia.

Drug interaction of Nitrendipine Powder.
(1) It can increase the plasma concentration of digitalis, such as digoxin, by about 45% on average.

Uses of Nitrendipine.

Pharmacological action of Nitrendipine.
Nitrendipine is used to treat hypertension, congestive heart failure, and hypertension with angina. It can selectively act on the calcium channels of vascular smooth muscle, inhibit the transmembrane calcium ion flow between vascular smooth muscle and myocardium, and has a greater affinity for blood vessels than myocardium, and a stronger selective effect on coronary arteries. The order of artery dilation intensity is coronary artery > femoral artery > renal artery > pulmonary artery. It can reduce myocardial oxygen consumption and protect ischemic myocardium. Product Method of Bulk Nitrendipine Powder.
The acid catalyzed condensation of m-nitrobenzaldehyde and ethyl acetoacetate gives 2-(3-nitrobenzylidene) ethyl acetoacetate. 2-(3-nitrobenzyl) ethyl acetoacetate and 3-aminobutyrate methyl ester were placed in Soxlet extractor flask, anhydrous ethanol was added, and the siphon of Soxlet extractor was filled with filter paper wrapped molecular sieve, and then placed overnight after being heated in a water bath and refluxed for 4h. The crystallization was recrystallized with 5 times the amount of anhydrous ethanol to obtain Nitrendipine pure product with melting point of 158 ~ 159℃ and yield of 90.8%. Without molecular sieve, the yield was 86.1%.
