    Pharmacological Effects of Omeprazole magnesium.

    Omeprazole is absorbed in the small intestine and is usually completely absorbed within 3-6 hours. Bioavailability after repeated administration is approximately 60%. Simultaneous ingestion of food has no effect on its bioavailability. Omeprazole has a plasma protein binding of 95% and an apparent volume of distribution of 0.3 L/kg.METABOLISM: Omeprazole is completely metabolised in the liver, mainly by CYP2C19 and CYP3A4 enzyme-catalysed metabolism. Its metabolites are sulfone, sulfide and hydroxy omeprazole, which have no significant effect on gastric acid secretion. Total plasma clearance is 0.3-0.6 L/min. omeprazole inhibits its own catalytic metabolism by CYP2C19, thus increasing the bioavailability of omeprazole by approximately 50% with multi-dose therapy compared to single-dose therapy. EXHAUSTION: The plasma elimination half-life of omeprazole is approximately 40 minutes (30-90 minutes). Approximately 80% of the metabolites are excreted in the urine and the remainder in the faeces. PATIENT FACTORS: The bioavailability of omeprazole is not significantly altered in elderly patients or patients with renal hypoplasia, and is elevated in patients with hepatic impairment, but clearance is significantly reduced in all of these patients

    1、Concomitant use with oral imidazole antifungal drugs such as ketoconazole, itraconazole, miconazole and fluconazole should be avoided.

    2, Omeprazole and clarithromycin co-administration can increase the incidence of central nervous system (mainly headache) and gastrointestinal adverse reactions.

    3、Concomitant use with diazepam (Valium), phenytoin, warfarin, nifedipine, digoxin, cisapride, quinidine, cyclosporine, caffeine and theophylline should be avoided.
