Dexamethasone (dexamethasone phosphate sodium EP impurity A) is a glucocorticoid used as an anti-inflammatory agent. Dexamethasone regulates the survival, growth and differentiation of T cells. Dexamethasone inhibits the induction of nitric oxide synthase.
It is mainly used as an anti-inflammatory and anti-allergic agent in rheumatoid arthritis and other collagenous diseases.
Uses and functions of Dexamethasone Powder.
(1) Oral acid-producing drugs reduce the gastrointestinal absorption of dexamethasone.
(2) Aminoglutethimide inhibits adrenocortical function, accelerating the metabolism of the drug and shortening its half-life. If glucocorticosteroids are used in combination with amiloride, hydrocortisone may be used.
(3) Dexamethasone may attenuate the effects of oral hypoglycemic agents, anticoagulants, and neuromuscular blocking agents, and the dose should be adjusted.
(4) Barbiturates, phenytoin, rifampicin will attenuate the metabolism promotion effect of dexamethasone.
(5) Dexamethasone increases the toxicity of salicylic acid.
(6) Dexamethasone acts antagonistically to vitamin A and should be avoided in combination.

A new process method for the production of dexamethasone 21-hydroxyl, using intermediate 21-acetate as a substrate, semi-solubilising the substrate with an appropriate amount of methanol containing 0-10% chloroform as a solvent, hydrolyzing the substrate with a base as a catalyst, neutralising the reaction with acetic acid after the reaction is complete, concentrating the reaction solution under reduced pressure to the appropriate volume, cooling down the temperature, filtering, rinsing the filter cake with water, and drying to obtain the 21-hydroxyl. This process can shorten the production cycle, improve the quality and yield of 21-hydroxy, and reduce the content of impurities in 21-hydroxy.
