Erythromycin (EM, Er) is produced by streptomycin and is an alkaline antibiotic. It includes Erythromycin A (ErA), Erythromycin B (ErB), Erythromycin C (ErC), Erythromycin D (ErD), Erythromycin E (ErE) and Erythromycin F (ErF). Its free base is for oral use and lactobionate is for injection. It belongs to the macrolide class of narrow-spectrum antimicrobials and is commonly used in penicillin-resistant Gram-positive bacterial infections and alternatives to penicillin allergy, with an antimicrobial spectrum similar to that of penicillin.

Erythromycin is a gastric actin receptor agonist. The dimethylamine group and the 14-membered lactone ring on the glycosidic chain of the erythromycin molecule have a similar spatial structure to that of gastric actin, which can act as a gastric actin receptor agonist and produce a mimetic gastric actin effect, inducing early onset of MMC phase III contractions in the fasting state and causing contractions of the gastric and duodenal smooth muscles.
The pro-gastric dynamic effect of erythromycin is dose-dependent. Low doses of erythromycin induce gastric contractions similar to those of spontaneous MMC, whereas high doses of erythromycin cause strong irregular gastro-duodenal contractions inducing vomiting, which is the reason why erythromycin induces gastro-intestinal reactions .
Therefore, erythromycin should not be used in excessive doses for gastric motility.
Clinically used in penicillin-resistant Gram bacterial infections and penicillin allergic patients, Legionella pneumonia, Mycoplasma pneumonia, Chlamydia trachomatis caused by infantile pneumonia and colitis, skin and soft tissue infections can be used as a drug of choice, and can be used for haemolytic streptococcus and pneumococcus caused by respiratory infections, diphtheria carriers, the efficacy of the product and diphtheria antitoxin combined with the enhancement of the efficacy of the drug.
